Dear Parents/Carers,

In 2025, the QLD Health School Immunisation Program run by Ozcare is moving to a digital consent system. This program, powered by Vitavo, will replace the paper consent card system that you may be familiar with from previous years. This will allow Ozcare to capture a digital profile and consent for your child/ren which covers their immunisations for the year. No more paper floating around school bags!

How it works:

  1. Click the below link, which will take you straight to the Vitavo Parent Portal for the Glenmore State High School students.
    1. https://ozcarecentralqld.book.vitavo.com.au/school/GlenmoreStateHighSchool-Dz7jMu
  2. Follow the prompts to create a Vitavo profile for your child, record their medical history and provide consent for the vaccines being administered for their year level.
  3. You will receive email communication throughout the process direct from the Vitavo system, with booking confirmation and pre-immunisation info, and post-vaccination confirmation accompanied by a side effects fact sheet.

Our 2025 School Immunisation Clinics are:

  • Year 7
    • HPV & dTpa Clinic | Monday 21 July 2025 – Periods 1 and 2
    • Catch-ups | Monday 1 September 2025 – Periods 1 and 2
  • Year 10
    • ACWY & MenB Dose 1 Clinic | Friday 28 March 2025 – Periods 1 and 2
    • MenB Dose 2 Clinic and Catch-ups | Monday 1 September 2025 – Periods 1 and 2

If families are struggling with the completion of this process digitally, we can provide the relevant paper consent cards for filling out and returning, as soon as possible.

Should you have any questions or concerns, please don't hesitate to contact us.

Kind Regards,

Brendan Shannon
Principal
